If covered by a spouse\u2019s policy, know your spouse\u2019s date of birth and Social Security number.<\/li>\n\n\n\n<li>A letter of authorization from your insurance carrier if your treatment is covered by <a href=\"https:\/\/www.uchealth.org\/services\/employer-solutions\/occupational-medicine\/\">Workers Compensation<\/a>.<\/li>\n\n\n\n<li>Previous medical records (new patients only).
