Maybe you are having heart palpitations off and on and are worried about that. Or you are having muscle spasms in your legs at night. Or a persistent cough that doesn\u2019t seem to be connected with a cold. Do you see your family medicine practitioner or do you head straight to an internal medicine specialist?<\/p>\n<p>Two doctors at the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-primary-care-rockrimmon\/\">UCHealth Primary Clinic \u2013 Rockrimmon<\/a> have some answers.<\/p>\n<h3><strong>Family medicine<\/strong><\/h3>\n<p><a href=\"https:\/\/www.uchealth.org\/provider\/george-meyers-md-family-medicine\/\">Dr. George Meyers<\/a> practices family medicine at the clinic.\u00a0\u00a0 But he\u2019s not hung up on terminology.<\/p>\n<p>\u201cI think that we just practice primary care,\u201d he said. Most people would define it as taking care of whole families, from newborns to geriatrics, \u201ccradle to grave,\u201d he added.\u00a0 \u201cWe deal with all diseases, all health issues.\u201d<\/p>\n<p>His background includes serving as an Army physician, remote medicine, emergency work and hospitalist work. He\u2019s seen it all. A few years ago, he settled into outpatient family practice with the primary care clinic in Rockrimmon.<\/p>\n<p>As a family physician, he experiences a lot of variety in his work. He gets simple to complicated cases at all age levels.<\/p>\n<figure id=\"attachment_24067\" aria-describedby=\"caption-attachment-24067\" style=\"width: 191px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-24067\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2019\/05\/16085822\/Capture1.webp\" alt=\"\" width=\"191\" height=\"247\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2019\/05\/16085822\/Capture1.webp 191w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2019\/05\/16085822\/Capture1-116x150.webp 116w\" sizes=\"auto, (max-width: 191px) 100vw, 191px\" \/><figcaption id=\"caption-attachment-24067\" class=\"wp-caption-text\">Dr. George Meyers<\/figcaption><\/figure>\n<p>\u201cThe nice thing about family medicine is we get to change up our schedule a lot more. I might treat a diabetic adult with kidney disease, then a child with an ear infection. The nice thing for me is the variety. It keeps you challenged, but also maintains skills in lots of different areas.\u201d<\/p>\n<p>He knows where his skills lie. He doesn\u2019t do deliveries anymore, but he does a variety of outpatient procedures including vasectomies.<\/p>\n<p>\u201cMostly, we just do what has to be done,\u201d he said, \u201cusing our best judgment and common sense.\u201d<\/p>\n<p>Though he doesn\u2019t have a \u201ctypical\u201d patient <em>per se,<\/em> he does have typical patients in each age category.<\/p>\n<p>For example, in the ages 18 months to 4 years old, the typical toddler comes in for wellness checks, vaccinations and the occasional illness. \u201cSometimes a parent wants their child to see the same family physician, especially if there is a special need, like autism.\u201d<\/p>\n<p>School-age kids into teens are usually seen for school and sports physicals, routine care, and the occasional illness.<\/p>\n<p>A typical young adult (ages 20-30) sometimes just wants a checkup or a look at specific medical concern.<\/p>\n<p>Typical patients from 40 to 60 years of age often are looking at evolving health issues, and seeking preventative medical care.<\/p>\n<p>By age 60 and older, some adults have developed chronic health issues that need more frequent attention, such as congestive heart failure, chronic lung disease or chronic kidney disease.<\/p>\n<p>He also treats sports injuries and has quite a bit of experience with that. \u201cHaving been an Army doctor, it\u2019s hard to avoid that,\u201d he said.<\/p>\n<p>\u201cI think the biggest thing for some people is knowing your entire family is being treated by the same person,\u201d he said. Sometimes, he finds himself treating four generations of the same family.<\/p>\n<p>Doing so gives him a better picture of that family\u2019s issues as well as health needs. He knows the caretakers and who\u2019s dealing with mental health issues that may affect others.\u00a0 He knows which grandparents are caring for grandkids and may be under stress.\u00a0 He has a feel for family dynamics.<\/p>\n<p>\u201cIt\u2019s also rewarding, because you get to assist the whole family through situations,\u201d he said.<\/p>\n<h3><strong>Internal medicine<\/strong><\/h3>\n<p><a href=\"https:\/\/www.uchealth.org\/provider\/laura-l-longwell-do-internal-medicine\/\">Dr. Laura Longwell<\/a>, a doctor of osteopathy specializing in geriatric care, practices in the same clinic with Dr. Meyers.<\/p>\n<p>\u201cWe are a clinic offering both internal medicine and family medicine \u2013 it did used to be all family medicine, but it changed a year ago when I joined the practice,\u201d she said.<\/p>\n<p>Patients ask all the time: \u201cWhat\u2019s the difference?\u201d<\/p>\n<figure id=\"attachment_21828\" aria-describedby=\"caption-attachment-21828\" style=\"width: 189px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-21828\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2019\/02\/06093947\/Dr.-Laura-Longwell.webp\" alt=\"A photo of Dr. Laura Longwell\" width=\"189\" height=\"253\" srcset=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2019\/02\/06093947\/Dr.-Laura-Longwell.webp 189w, https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2019\/02\/06093947\/Dr.-Laura-Longwell-112x150.webp 112w\" sizes=\"auto, (max-width: 189px) 100vw, 189px\" \/><figcaption id=\"caption-attachment-21828\" class=\"wp-caption-text\">Dr. Laura Longwell<\/figcaption><\/figure>\n<p>So, she explains.<\/p>\n<p>For one thing, she only treats adult patients, 18 and older, and in fact most of her patients are middle aged (40-50) and older.<\/p>\n<p>\u201dI\u2019m not a geriatrician but I am interested in geriatrics,\u201d she said. \u201cAnd by the very nature of what I treat, many of my patients are older.\u201d Her patient profile is mostly defined by the types of issues and illnesses she treats \u2013 persistent or chronic medical concerns, like diabetes, hypertension, high cholesterol, heart disease, congestive heart failure or chronic kidney disease.<\/p>\n<p>These are illnesses that often affect patients the rest of their lives, \u201cbut I try to be positive. Instead of saying there\u2019s no cure, I tell them, \u2018We can manage this and you can have a good life, Mindset really makes a huge difference. You can have chronic diseases like <a href=\"https:\/\/www.uchealth.org\/diseases-conditions\/copd-chronic-obstructive-pulmonary-disease\/\">COPD<\/a> or heart disease, but that doesn\u2019t mean you\u2019re going to have a miserable life.\u201d<\/p>\n<p>Her biggest frustration, she said, is overcoming a previous thought process put into place by a different health care provider. She has had patients who rejected health care for too long because they were given discouraging news by another provider.<\/p>\n<p>I think (we) should be mindful about what we are telling a patient,\u201d she said. \u201cWe\u2019re supposed to be kind and caring people. Being kind can go a long way toward being therapeutic.\u201d<\/p>\n<p>After finishing medical school she did three years post-doctoral training as a resident at a hospital, where she spent a lot of time caring for patients in intensive care. She worked with many specialists in cardiology, gastroenterology, nephrology, infectious diseases, hematology, auto-immune diseases, pulmonary and critical care.<\/p>\n<p>\u201cYou get used to pretty sick people, and see the worst-case scenario of many diseases,\u201d she said, \u201cso I have a great deal of background in all those areas.\u201d<\/p>\n<p>An internist is \u201ckind of like a health care coordinator. You can manage all those things, order some of the diagnostics and interpret some of them, but when you put the pieces together, you sometimes have to refer patients to specialists,\u201d she said. \u201cYou need to have a healthy knowledge of your limitations, too.\u201d<\/p>\n<p>She also tries to be cognizant of health care costs.<\/p>\n<p>\u201cIf I think I\u2019m going to refer a patient, I don\u2019t do a lot of workups,\u201d she said. \u201cThey\u2019ll just be repeated.\u201d<\/p>\n<h3><strong>The bottom line<\/strong><\/h3>\n<p>So which doctor do you see?<\/p>\n<p>\u201cHonestly, I don\u2019t think there\u2019s a real discriminator,\u201d Meyers said. \u201cIt just comes down to preference and fit. It\u2019s really hard to say.\u00a0 A few years ago, internists mainly did hospitalist work. That\u2019s no longer true.\u201d<\/p>\n<p>Longwell said: \u201cInternal medicine has the benefit of focusing just on adult care, which allows us to go a little deeper into these medical problems.\u201d And she adds: \u201cI have to give family medicine physicians a whole out of credit as they take care of everybody and that is a vast amount of information to know.\u201d<\/p>\n<p>In other words, it\u2019s really up to the patient to decide. <a href=\"https:\/\/www.uchealth.org\/schedule-appointment\/\">Schedule an appointment<\/a> online.<\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>When you think you need to go to the doctor, do you know which doctor to go to?
