But the diseases have one common thread: inflammation.<\/p>\n<p>\u201cRheumatology is a confusing specialty to many people, but the easiest way to think about it is that we\u2019re the doctors of inflammation,\u201d said <a href=\"https:\/\/www.uchealth.org\/provider\/nicole-cotter-md\/\" target=\"_blank\" rel=\"noopener noreferrer\">Dr. Nicole Cotter<\/a>, a rheumatologist with <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-rheumatology-clinic-steamboat-springs\/\" target=\"_blank\" rel=\"noopener noreferrer\">UCHealth Rheumatology Clinic in Steamboat Springs<\/a>.<\/p>\n<p>Rheumatologists manage \u2018systemic autoimmune diseases,\u2019 or diseases that affect many organ systems and result from the immune system attacking a person\u2019s own body.<\/p>\n<p>\u201cIt\u2019s like the immune system gets confused,\u201d Cotter said. \u201cIt starts to create inflammation in organs where there\u2019s not a problem, and because of that inflammation, damage can occur.\u201d<\/p>\n<p>The list of diseases that rheumatologists diagnose and treat includes rheumatoid arthritis, lupus, inflammatory myopathies, vasculitis, gout, Sjogren\u2019s syndrome (dry eyes and dry mouth) and osteoarthritis.<\/p>\n<p>As with other autoimmune diseases, it isn\u2019t clear what causes rheumatological diseases. But during the extensive physical exam Cotter conducts, in which she also takes a detailed patient history and reviews family history, she sometimes finds other symptoms.<\/p>\n<p>\u201cWe might pick up on things the patient doesn\u2019t realize are going on,\u201d Cotter said. \u201cThey may come in for a swollen joint, then I\u2019ll say, \u2018What about this rash behind your ear?\u2019 Or, I\u2019ll ask if they\u2019ve had muscle weakness and they\u2019ll say, \u2018Yes, actually, over the past few years it\u2019s been hard getting out of my chair.\u201d<\/p>\n<p>It\u2019s important not to ignore symptoms of any rheumatological disease, as early treatment is key. For instance, data shows that people with rheumatoid arthritis who are diagnosed early and treated aggressively do better over the long-term.<\/p>\n<p>\u201cEarly diagnosis and early intervention is of the utmost importance,\u201d Cotter said. \u201cA lot of times people with joint pain will think, \u2018I\u2019m getting older,\u2019 or \u2018I was just aggressive with my workout yesterday.\u2019 But I\u2019d rather someone get checked out and find nothing is wrong, than for someone to wait two years and, by the time the issue is found, learn there\u2019s already been damage to the joint.\u201d<\/p>\n<p>Lab tests alone cannot diagnosis or exclude a rheumatological disease. Someone may have antibodies that signal a possible issue and not have a disease, while someone else with a normal lab test may actually have a disease.<\/p>\n<p>\u201cThere isn\u2019t one thing that can tell me \u2018yes\u2019 or \u2018no,\u2019\u201d Cotter said. \u201cIt really is putting a puzzle together and getting enough evidence that there\u2019s a problem.\u201d<\/p>\n<p>When treating rheumatological diseases, the goal is management and remission. Medications play an important role in helping to suppress the abnormal immune response.<\/p>\n<p>\u201cThe last 20 years have seen the advent of biologic drugs, which are targeted therapies that work on specific parts of the inflammation pathways,\u201d Cotter said. \u201cThey\u2019ve revolutionized the management of these diseases.\u201d<\/p>\n<p>Lifestyle changes are also key, including adjusting diets and reducing stress.<\/p>\n<p>And while a diagnosis can feel daunting, there is hope.<\/p>\n<p>\u201cPeople with rheumatoid arthritis can come in feeling awful \u2013 their joints are swollen, they have trouble walking and doing daily tasks, they\u2019re tired and feel wiped out. A lot of people are afraid that this is going to be their life,\u201d Cotter said. \u201cBut we absolutely have treatments to get people feeling better and functioning better.
