{"id":62899,"date":"2022-03-10T16:00:31","date_gmt":"2022-03-10T23:00:31","guid":{"rendered":"https:\/\/www.uchealth.org\/today\/?p=62899"},"modified":"2022-11-16T10:58:05","modified_gmt":"2022-11-16T17:58:05","slug":"emergency-care-nearby-saves-woodland-park-man","status":"publish","type":"post","link":"https:\/\/www.uchealth.org\/today\/emergency-care-nearby-saves-woodland-park-man\/","title":{"rendered":"Having emergency care nearby saves Woodland Park man"},"content":{"rendered":"<div style=\"margin-top: 0px; margin-bottom: 0px;\" class=\"sharethis-inline-share-buttons\" ><\/div><p>It was the middle of the night, and 68-year-old Mike Welch was a little hesitant to wake his wife, but he remembered the advice his daughter-in-law, a nurse, had given him a couple of weeks prior when he told her he was having some concerns about his heart: \u201cWell, you don\u2019t want to fool around \u2013 go and get it checked out,\u201d he recalled her telling him.<\/p>\n<p>\u201cIn my mind, it was a little foolish to wake my wife up at 3 a.m. in the morning,\u201d he said. He really wasn\u2019t in any great pain. But when his wife, Lezlee, heard what he had to say \u2013 just four words \u2013 she was wide awake. \u201cHe said, \u2018I don\u2019t feel right,\u2019 and the word \u2018right\u2019 just didn\u2019t sound right coming from him,\u201d she said. \u201cIt clued me in something was terribly wrong.\u201d<\/p>\n<p>Mike\u2019s left shoulder was aching and he was short of breath.<\/p>\n<figure id=\"attachment_62904\" aria-describedby=\"caption-attachment-62904\" style=\"width: 640px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-62904\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2022\/03\/10152834\/Mike-and-Lezlee-Welch3.webp\" alt=\"Mike and Lezlee Welch at their home. Photo: UCHealth.<\/figcaption><\/figure>\n<p>The couple got in the car and Lezlee headed toward <a id=\"\" href=\"https:\/\/www.uchealth.org\/locations\/uchealth-pikes-peak-regional-hospital\/\">UCHealth Pikes Peak Regional Hospital<\/a>, about 5 or 6 minutes away from their Woodland Park home. They passed a fire station in their own neighborhood on the way, but Mike urged his wife to keep going. \u201cHe said, \u2018drive faster \u2026 I want to go to the hospital.\u2019\u201d He\u2019d repeat those two words again to his wife \u2013 more urgently &#8211; and that was the last memory he\u2019d have for several days.<\/p>\n<p>Mike\u2019s heart stopped seconds after they arrived at the emergency department. Photo: UCHealth.<\/figcaption><\/figure>\n<p>On duty that night \u2013 Feb. 1, 2022, &#8211; was Dr. Miguel Sandoval, an emergency medicine physician, who along with two other nurses and a tech, quickly began trying to revive Mike.<\/p>\n<p>\u201cAt that point, he looked like he had no pulse. We got him on the gurney, I checked for a pulse, and then I started compressions. He was in what\u2019s called ventricular fibrillation, and it\u2019s one of those rhythms that you see frequently in folks that are having cardiac problems; it\u2019s a deadly rhythm.<\/p>\n<p>\u201cAfter we got him on the monitor, we did defibrillate him, and then we continued another two minutes of CPR,\u201d Sandoval recalled.<\/p>\n<p>Mike\u2019s heart began beating, but he was by no means out of the woods. He needed medication and fluids, but the team was having trouble placing an IV.<\/p>\n<p>\u201cOnce we had a pulse, we still did not have any way to give him medications. I then put in an intraosseous line &#8211; basically, I drilled in a metal IV, a needle into his shin bone, in order to give him that. It\u2019s a common procedure that we will do if we have a hard time getting access on folks quickly,\u201d Sandoval said. \u201cWe started giving him some fluids and I gave him a medication called epinephrine, or adrenaline, to help prevent him from going back into arrest.\u201d<\/p>\n<p>Mike\u2019s blood pressure was low, and Sandoval was worried at that moment that Mike\u2019s heart would again stop beating.<\/p>\n<p>The next priority was to get Mike to <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-memorial-hospital-central\/\">UCHealth Memorial Hospital Central<\/a> in Colorado Springs for further cardiac care. The team called for helicopter transport.<\/p>\n<p>\u201cFrom a critical access hospital in Woodland Park, the next step is you need to go to an ICU,\u201d Sandoval said.<\/p>\n<p>At Memorial Central, Mike was given medication to stabilize his blood pressure and was taken to the cath lab. <a href=\"https:\/\/www.uchealth.org\/provider\/cihan-cevik-md\/\">Dr. Cihan Cevik<\/a>, an interventional cardiologist, was on call early that morning.<\/p>\n<p>\u201cHis LAD (left anterior descending artery) was 90% blocked,\u201d said Cevik, who guided a catheter up through a wrist artery to the blockage, where he then placed a stent to keep the blood vessel open.<\/p>\n<p>The procedure took 22 minutes, and Cevik went to see Welch the next day in the ICU.<\/p>\n<p>\u201cHe said he was feeling great \u2013 that he was ready to go home,\u201d Cevik recalled.<\/p>\n<p>Sandoval also called Memorial Hospital to check on his patient the next day. He was thrilled with what he learned: Mike was doing well, with no neuro deficits.<\/p>\n<p>Sandoval also said that while he always encourages everyone to consider 911 in an emergency, in this case, the timing could not have been better for Mike. \u201cThe fact that they were here, at the hospital, and he had a downtime of maybe three minutes before getting CPR, defibrillation, and epinephrine &#8211; that\u2019s mind-boggling to think about how fortunate we were with the timing. Cardiac arrest in general, the outcomes are not that great; it&#8217;s not television. For him to have arrested and gotten a return of circulation with a pulse as quickly as he did was awesome.\u201d<\/p>\n<p>Lezlee Welch wants everyone to know about the gem of a hospital that\u2019s in their community so much so that she posted the details of Mike\u2019s cardiac arrest and his care at Pikes Peak Regional on Facebook.<\/p>\n<p>\u201cEveryone in Woodland Park needs to know &#8211; the older people especially &#8211; that the hospital can handle critical emergencies. People need to realize it\u2019s a great hospital &#8211; they can handle it. Have the confidence that in an emergency you can go there; you don\u2019t have to drive down the hill (to Colorado Springs.)<\/p>\n<p>\u201cBack a long time ago, he probably would still be in the hospital, but the amazing part of this whole thing is you can actually go into cardiac arrest completely have your heart stop on a Tuesday morning and be home by Friday and it was just amazing,\u201d she added.<\/p>\n<p>She and Mike are also making some lifestyle changes, particularly to their diet. Photo: UCHealth.<\/figcaption><\/figure>\n<p>\u201cAnother thing that I think is very important for us &#8211; for everybody &#8211; is to <a id=\"\" href=\"https:\/\/www.redcross.org\/take-a-class\/cpr\/cpr-training\" target=\"_blank\" rel=\"noopener\">take a CPR class<\/a>. Had I been here (at home), and he had lost consciousness here, I would\u2019ve been in a lot of trouble,\u201d Lezlee said. \u201cSo that\u2019s something we\u2019re going to do together. I\u2019ve always wanted to; I have grandchildren I need to know.\u201d<\/p>\n<p>Sandoval can only smile when he thinks of that night at Pikes Peak Regional Hospital \u2013 a place he loves to work.<\/p>\n<p>\u201cThe last few years have been not awesome for medicine, and definitely not emergency medicine,\u201d he said, referring to the COVID-19 pandemic. \u201cYou know there\u2019s been a lot of mistrust and anger and sadness. At that moment, Mr. Welch needed us, but I also needed this.
