Photo by Cyrus McCrimmon for UCHealth.<\/figcaption><\/figure>\n<p>Whatever the stereotype of a hobbyist cross-stitcher may be, Ray Henderson does not fit it. Henderson, 65, is an electrical engineer who specializes in control systems for Los Alamos National Laboratory. But he finds the methodical, exacting process of stitching square after tiny square with colored thread to be soothing, and its colorful, tactile products to be a satisfying complement to the detailed digital renderings he produced in his day job.<\/p>\n<p>Over the years, he finished cross-stitched Christmas-stockings for his three grown children and his first grandchild, each taking hundreds of hours to complete. Then came grandchild number two, a baby girl named Esmeralda. She would not, it seemed, be getting a cross-stitched stocking of her own.<\/p>\n<p>Henderson was diagnosed with Parkinson\u2019s disease in mid-2018. By 2021, it eroded his fine-motor skills to below the cross-stitching threshold while he was working on Esmeralda\u2019s stocking. Parkinson\u2019s also affected his walking, overall energy, and the quality of his sleep. What had been \u201c<a id=\"\" href=\"https:\/\/link.springer.com\/chapter\/10.1007\/978-1-60327-426-5_7\" target=\"_blank\" rel=\"noopener\">on-off<\/a>\u201d cycles \u2013 \u201con\u201d being when his <a id=\"\" href=\"https:\/\/medlineplus.gov\/druginfo\/meds\/a601068.html\" target=\"_blank\" rel=\"noopener\">carbidopa-levodopa<\/a> pills helped him feel like his old self \u2013 were now off-on cycles: The three hours \u201con\u201d and one hour \u201coff\u201d between doses had degraded to the opposite. He had to schedule trips to the supermarket near his home north of Longmont to be back home before the drugs wore off.<\/p>\n<p>\u201cI\u2019d think, OK, I\u2019ll take my medicines now, I\u2019m going to be \u2018on\u2019 in 20-30 minutes, and I can go to the grocery store,\u201d Henderson said. \u201cIf I can get the grocery store done in 30 minutes, I can get home before I go \u2018off\u2019.\u201d<\/p>\n<h2><strong>Special delivery via Vyalev<\/strong><\/h2>\n<p>No longer, thanks to a medical device that the U.S. Food and Drug Administration <a id=\"\" href=\"https:\/\/news.abbvie.com\/2024-10-17-U-S-FDA-Approves-VYALEV-TM-foscarbidopa-and-foslevodopa-for-Adults-Living-with-Advanced-Parkinsons-Disease\" target=\"_blank\" rel=\"noopener\">approved on Oct. 17<\/a>. Known as AbbVie-951 when Henderson started using it two years ago, it now goes by Vyalev. It\u2019s a pump made by AbbVie that dispenses a formulation of foscarbidopa and foslevodopa continuously through a small needle rather than in discrete, pill-delivered doses. Carbidopa and levodopa combine to deliver the neurotransmitter dopamine to Henderson\u2019s brain \u2013 dopamine that <a href=\"https:\/\/www.uchealth.org\/diseases-conditions\/parkinsons-disease\/\">Parkinson\u2019s disease<\/a> robs its hosts of. A lack of dopamine is complicit in many Parkinson\u2019s symptoms.<\/p>\n<p>University of Colorado School of Medicine movement-disorders neurologist <a href=\"https:\/\/www.uchealth.org\/provider\/drew-kern-md\/\">Dr. Drew Kern<\/a> has led three <a id=\"\" href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT03781167?term=ABBV-951&amp;cond=Parkinson&amp;cntry=US&amp;draw=2&amp;rank=1\" target=\"_blank\" rel=\"noopener\">clinical trials<\/a> and is continuing to run three long-term extension studies of the AbbVie device at the CU School of Medicine\u2019s Clinical and Translational Research Center (<a id=\"\" href=\"https:\/\/cctsi.cuanschutz.edu\/resources\/ctrc\" target=\"_blank\" rel=\"noopener\">CTRC<\/a>) site at UCHealth University of Colorado Hospital (<a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-university-of-colorado-hospital-uch\/\">UCH<\/a>) on the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-at-university-of-colorado-anschutz-medical-campus\/\">Anschutz Medical Campus<\/a>. Henderson joined the third trial, taking his first dose on Nov. 1, 2022. Photo by Cyrus McCrimmon for UCHealth.<\/figcaption><\/figure>\n<p>Kern says AbbVie developed a new, pH-neutral liquid formulation of carbidopa\/levodopa for the device, but it\u2019s the consistent delivery of these proven drugs that makes the difference. He likened the subcutaneous needle-pump combination to the <a href=\"https:\/\/www.uchealth.org\/services\/diabetes-endocrinology-care\/diabetes-management\/\">insulin pumps<\/a> that have revolutionized diabetes care. The idea of continuous medication delivery wasn\u2019t a new idea in Parkinson\u2019s treatment, he says. AbbVie had already developed a carbidopa-levodopa gel to be gradually dispensed directly into the gut. But that approach has drawbacks.<\/p>\n<p>\u201cIt\u2019s a feeding tube, and it\u2019s cumbersome, and it requires surgery,\u201d Kern said. \u201cA lot of Coloradans don\u2019t want to walk around with a feeding tube if they\u2019re hiking, camping, and everything else.\u201d<\/p>\n<h2><strong>Continuous carbidopa-levodopa flow for an \u2018even keel\u2019<\/strong><\/h2>\n<p>The Vialev pump dispenses the foscarbidopa\/foslevodopa liquid through a needle affixed with a small bandage \u2013 no surgery required. The pH-neutral formulation was important in minimizing skin irritation, Kern says. The bandage and its injection site get moved to a new spot every two to three days.<\/p>\n<p>The pump dispenses 10 milliliters of the drug combination from a syringe inside the device. Henderson replaces it once a day. Within set parameters, he can adjust the dosage with what amount to \u201chigh,\u201d \u201cmedium,\u201d and \u201clow\u201d settings. Photo by Cyrus McCrimmon for UCHealth.<\/figcaption><\/figure>\n<p>\u201cIt puts me on an even keel,\u201d Henderson said. \u201cI\u2019m still doing things I couldn\u2019t do without the pump.\u201d<\/p>\n<p>He feels good, he says. He has no problem getting up at night or in the morning \u2013 previously a struggle because his oral medications had worn off. He\u2019s not watching the clock when he runs errands. His wife Kathy doesn\u2019t have to cut his food for him. He walks Tillie, their cocker spaniel, a mile every day.<\/p>\n<p>\u201cI don\u2019t really have \u2018off\u2019 times anymore,\u201d Henderson said. \u201cI\u2019m pretty much \u2018on\u2019 all the time, 24\/7, though some \u2018ons\u2019 are better than others.\u201d<\/p>\n<h2><strong>Pump is a \u2018game changer\u2019\u00a0<\/strong><\/h2>\n<p>Kern is bullish on the device\u2019s potential to serve patients with more advanced Parkinson\u2019s disease (pills generally work well until the disease progresses, he says). The pump enables fine-tuned dosing that\u2019s hard to achieve with pills. It can provide an option for patients who aren\u2019t interested in or, given their age or health status, makes <a href=\"https:\/\/www.uchealth.org\/today\/dbs-for-parkinsons-helps-young-rocket-scientist-fight-worst-parkinsons-symptoms\/\">deep-brain stimulation neurosurgery<\/a> too high-risk.<\/p>\n<p>\u201cBy the time somebody\u2019s five or 10 years into the disease course, there\u2019s a high likelihood they would benefit from this treatment,\u201d Kern said.<\/p>\n<p>Kern sees the AbbVie pump as a \u201cgame changer\u201d for many among the 1 million people with Parkinson\u2019s disease in the United States. He says Vyalev\u2019s FDA approval has already brought inquiries from fellow neurologists, and that he expects general neurologists as well as primary care physicians to ultimately prescribe the pump.<\/p>\n<figure id=\"attachment_79129\" aria-describedby=\"caption-attachment-79129\" style=\"width: 400px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-79129\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2024\/11\/25093719\/Ray-Henderson-cross-stitch-edsized.webp\" alt=\"The cross-stitching for Esmeralda\u2019s Christmas stocking as it neared completion. Photo courtesy of Ray Henderson.<\/figcaption><\/figure>\n<p>The benefits to Henderson have been clear: improved sleep, improved mobility, and improved fine-motor skills. Having the pump as a constant companion took some getting used to, he said, and it weighs on him at times. But he no longer must set an iPhone reminder to take carbidopa-levodopa pills every four hours \u2013 reminders he sometimes missed, paying the price in more \u201coff\u201d time. The pump also keeps running at night, avoiding gaps during sleep.<\/p>\n<p>As a long-term Vyalev user, he and Kathy have become expert in changing the location of the cannula to different spots in his abdomen, doing so in about five minutes. He likens the cannula to a thumbtack, its nine-millimeter (0.35-inch) needle which delivers the medication. Switching out the medication syringe also takes about five minutes, he says. The pump reminds him when it\u2019s time to do that and, occasionally, replace the pump\u2019s battery. He has learned that, if skin near the cannula seems irritated, to move it to another spot.<\/p>\n<p>Henderson knows that the AbbVie pump isn\u2019t a cure \u2013 as with carbidopa-levodopa pills, it manages symptoms, not the underlying Parkinson\u2019s disease.<\/p>\n<p>\u201cThe Parkinson\u2019s is still there. I still have all the things that go with Parkinson\u2019s, he said. \u201cBut what I don\u2019t have is the \u2018off\u2019 time of having to deal with Parkinson\u2019s.\u201d<\/p>\n<p>Among the fruits of Henderson saying \u201con\u201d?
