Photo: Getty Images.<\/figcaption><\/figure>\n<p>Millions of people in the U.S. who are ages 65 and older are officially considered &#8220;frail,&#8221; and due to demographic trends, the number of vulnerable people is rising fast.<\/p>\n<p>Being frail means that you&#8217;re physically or mentally compromised to the point that you are much more vulnerable to suffering additional health problems.<\/p>\n<p>More than a decade ago, the National Health and Aging Trends Study <a id=\"\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4723664\/#:~:text=Results.,11%25%20of%20persons%20considered%20robust.\" target=\"_blank\" rel=\"noopener\">estimated<\/a> that 15% of older adults in the U.S., who were at least age 65 and not living in nursing homes, were already frail.<\/p>\n<p>The 2015 estimate equated to roughly 6 million people, but as the <a id=\"\" href=\"https:\/\/www.census.gov\/content\/dam\/Census\/library\/stories\/2023\/05\/2020-census-united-states-older-population-grew-figure-1.jpg\" target=\"_blank\" rel=\"noopener\">nation\u2019s population has aged<\/a>, so has the number of people in this age group who now are considered frail. Today, the estimate has climbed to 8 million people.<\/p>\n<p>Mere statistics hide the significance of this rise in the number of frail older adults. In medical and human terms, frailty is much more than physical weakness. It is a collection of problems that erode the bodily reserves that enable a person to fight disease, injury, trauma and illness. Frailty takes a toll on individuals, caregivers and the health care system as a whole.<\/p>\n<p>Frailty is not the burden of older people alone. Nor is it the inevitable result of aging. People can take steps to prevent or reverse frailty, and doing so represents a huge step forward, both for the individual and for society.<\/p>\n<p>To learn more about the challenges of frailty, we spoke with <a href=\"https:\/\/www.uchealth.org\/provider\/thomas-johnson-md\/\">Dr. Thomas Johnson<\/a>, assistant professor in the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/geriatrics\" target=\"_blank\" rel=\"noopener\">Division of Geriatric Medicine<\/a> at the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/\" target=\"_blank\" rel=\"noopener\">University of Colorado Anschutz School of Medicine<\/a>. Johnson treats patients in the <a href=\"https:\/\/www.uchealth.org\/locations\/uchealth-seniors-clinic-anschutz-outpatient-pavilion\/\">UCHealth Seniors Clinic \u2013 Anschutz Outpatient Pavilion<\/a>.<\/p>\n<h2><strong>How exactly do you define medical frailty?<\/strong><\/h2>\n<p>Johnson began by quoting from a <a id=\"\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5726589\/#:~:text=Fried%20and%20Walston%20proposed%20the,low%20activity%20and%20weight%20loss.&amp;text=Fried%20and%20colleagues%20defined%20frailty,it%20using%20the%20frailty%20phenotype.&amp;text=The%20deficit%20accumulation%20approach%20emerged,domains%20to%20determine%20frailty%20status.&amp;text=More%20recently%2C%20a%20geriatric%20clinic,factors%2C%20along%20with%20physical%20components.&amp;text=In%202013%2C%20members%20of%20a,dependency%20and\/or%20death.%E2%80%9D\" target=\"_blank\" rel=\"noopener\">2019 paper<\/a> that defines frailty \u201cas a biologic syndrome of decreased reserve and resistance to stressors, resulting from cumulative declines across multiple physiologic systems, and causing vulnerability to adverse outcomes.\u201d<\/p>\n<p>In plainer terms, he said, \u201cpeople who are frail are more at risk for worse outcomes and have more difficulty returning to their normal previous function after a stressor.\u201d<\/p>\n<p>Those stressors include falls, injuries, hospitalizations, illnesses and chronic diseases.<\/p>\n<h2><strong>Is frailty a condition that is limited to older adults?<\/strong><\/h2>\n<p>No, frailty is not a condition only of older people. Frailty can occur at any age in people with many illnesses, ranging from chronic infections to end-stage kidney disease to cancer, Johnson said.<\/p>\n<figure id=\"attachment_90026\" aria-describedby=\"caption-attachment-90026\" style=\"width: 250px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-90026\" src=\"https:\/\/uchealth-wp-uploads.s3.amazonaws.com\/wp-content\/uploads\/sites\/6\/2026\/04\/21110359\/Frailty-q-and-a-1-Thomas-Johnson-web.webp\" alt=\"Dr. Thomas Johnson, assistant professor in the Division of Geriatric Medicine at the University of Colorado Anschutz School of Medicine. Photo coutesy of the University of Colorado Anschutz.<\/figcaption><\/figure>\n<h2><strong>Is frailty inevitable as we age?<\/strong><\/h2>\n<p>No, frailty is not inevitable.<\/p>\n<p>\u201cPeople may assume that every adult as they become older will become frail,\u201d Johnson said. \u201cThat\u2019s not true. It\u2019s something that we can try to prevent and fight against with a healthy lifestyle, exercise and social engagement.\u201d<\/p>\n<p>We\u2019ll provide more information on preventive steps you can take in a moment.<\/p>\n<h2><strong>Can a person\u2019s chronological and biological age be two different things?<\/strong><\/h2>\n<p>Yes, a person\u2019s chronological age may not predict their biological age, and vice versa, Johnson said.<\/p>\n<p>\u201cWe\u2019ve all met people who are, say, age 72 and we\u2019re surprised,\u201d he said. \u201cMaybe they are youthful and very active, or maybe they aren\u2019t and you would guess they are older. We can\u2019t help our chronological age, but we can influence our biological age with a healthy lifestyle and working with our providers to treat our medical illnesses.\u201d<\/p>\n<p>In working as a doctor with \u201cyoung learners,\u201d Johnson said he explains that he doesn\u2019t care nearly as much about a patient\u2019s age in years as he does about their function and whether that indicates if they are frail or on the cusp of becoming frail.<\/p>\n<h2><strong>How do you decide if a person is frail?<\/strong><\/h2>\n<p>There are <a id=\"\" href=\"https:\/\/frailtyscience.org\/wp-content\/uploads\/2020\/12\/Frailty-Assessment-Definition-Sheets-combined-2.pdf\" target=\"_blank\" rel=\"noopener\">five criteria<\/a> that medical experts use to determine whether or not a person is frail or at risk for becoming frail, Johnson said.<\/p>\n<h3><strong>The five criteria that determine if a person is frail include:<\/strong><\/h3>\n<ul>\n<li>Unintentional weight loss in the last year.<\/li>\n<li>Weakness, as measured by grip strength.<\/li>\n<li>Low energy and endurance, as measured by how fatigued a person gets after certain activities.<\/li>\n<li>Slowness, as measured by walking speed.<\/li>\n<li>Low physical activity, which is estimated by how many calories a person expends..<\/li>\n<\/ul>\n<p>A person who is frail meets at least three of these criteria, Johnson said. A person who meets one or two is at risk of becoming frail.<\/p>\n<p>\u201cIt is super important that we recognize these risks early and intervene,\u201d Johnson said.<\/p>\n<h2><strong>What are the potential consequences for people who are frail?<\/strong><\/h2>\n<p>\u201cThere have been studies that have looked into this,\u201d Johnson said. \u201cDoes frailty matter in the real world? Does it change people? It really does.\u201d<\/p>\n<p>For example, <a id=\"\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11369349\/#:~:text=Frailty%20is%20a%20multidimensional%20syndrome,and%20intervene%20during%20early%20stages.\" target=\"_blank\" rel=\"noopener\">one study<\/a> looked at the possible connections between frailty and disease processes. It noted that patients who are frail are at greater risk for developing chronic diseases like high blood pressure, diabetes, chronic kidney disease, heart disease and anemia. In turn, people with these diseases are at greater risk of becoming frail.<\/p>\n<p>The authors concluded that \u201cfrailty and <a href=\"https:\/\/www.uchealth.org\/today\/how-older-adults-can-manage-chronic-conditions\/\">chronic diseases<\/a> go hand in hand and it is of utmost importance to identify them and intervene during early stages.\u201d<\/p>\n<p>People who are frail frequently require higher levels of care, Johnson said.<\/p>\n<p>\u201cThey are more likely to need nursing home care, hospitalizations and longer hospital stays,\u201d he noted.<\/p>\n<h2><strong>Is frailty an important consideration for people who want to have surgery or require surgery? <\/strong><\/h2>\n<p>Yes. It is very important to think about frailty in surgery and surgical planning, Johnson said.<\/p>\n<p>The goal is to prepare people as well as possible to withstand and recover from the rigors of surgery.<\/p>\n<p>One way to do that is through \u201cprehabilitation\u201d with a physical therapist before the procedure. Patients may also get nutritional and psychological support before a surgery.<\/p>\n<p>The UCHealth Seniors Clinic offers physical therapy for its patients before surgery, Johnson noted.<\/p>\n<p>\u201cPatients with preoperative paperwork can walk down the hall to a PT (physical therapist) and get started instead of having to wait for an appointment,\u201d Johnson said.<\/p>\n<p>Evidence shows that people who get physical therapy before surgery have better outcomes, but it\u2019s not a guarantee. For example, one major <a id=\"\" href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/0284186X.2016.1268268#d1e747\" target=\"_blank\" rel=\"noopener\">study<\/a> of colorectal cancer patients facing surgery showed no difference in outcomes between those who had a prehabilitation program versus those who had standard care. However, those prehabilitation patients who attended at least 80% of their visits showed improvements in their complication rates and in their six-minute walking times.<\/p>\n<p>The takeaway from the study is important, Johnson said.<\/p>\n<p>\u201cPatients who are motivated and put in the work will have better outcomes from surgery.\u201d<\/p>\n<h2><strong>Are there psychological and social consequences of frailty?<\/strong><\/h2>\n<p>Yes. Frailty frequently causes psychological and social impacts.<\/p>\n<p>\u201cPeople who are frail are less active and are exhausted when they move,\u201d Johnson said. \u201cThis can lead to social isolation. In senior living (centers), people may decide to take all their meals in their room and not leave their room anymore. In the community, frailty can limit a person\u2019s ability to engage in the things they love, like hobbies and having coffee with friends.\u201d<\/p>\n<p>In these ways, frailty creates \u201ca vicious cycle,\u201d Johnson said.<\/p>\n<p>As people become more and more fatigued, they stop exercising, which in turn causes muscles to deteriorate. As they become more frail, their ability to move around declines, and they become more sedentary and more isolated.<\/p>\n<p>Johnson emphasized that social isolation is strongly related but not exclusive to frailty. It is important to identify its causes for anyone who experiences it. These can include transportation problems, financial issues, depression or problems with mobility.<\/p>\n<p>The good news is that antidotes for loneliness, apathy and loss of purpose are at hand for people who exercise and get out to experience nature, art and other social activities, Johnson said.<\/p>\n<p>\u201cBy engaging in these behaviors, we can improve our quality of life and quantity of life as well,\u201d he said.<\/p>\n<h2><strong>What are some individual and societal benefits of preventing or reversing frailty?<\/strong><\/h2>\n<p>\u201cPreventing frailty reduces the costs of care and hospitalizations, but it also helps people to live the lives they want,\u201d Johnson said. \u201cA lot of my patients want to avoid living in a nursing home, and the way that they can do that is by preventing frailty.\u201d<div class=\"su-pullquote su-pullquote-align-right\">UCHealth&#8217;s <a href=\"https:\/\/www.uchealth.org\/services\/community-health\/aspen-club\/\">Aspen Club provides health-related programs and services<\/a> to those 50 and forward in Larimer and Weld Counties to help older adults age healthfully and independently<\/div>\n<p>The challenge of preventing frailty is heightened by the <a id=\"\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC1464018\/#:~:text=A%20major%20public%20policy%20concern,different%20from%20macro%20cost%20issues.\" target=\"_blank\" rel=\"noopener\">aging of the Baby Boomer generation<\/a>, which threatens \u201cto massively shift costs if we don\u2019t age in healthy ways,\u201d Johnson said.<\/p>\n<p>\u201cIt will be very difficult for our health system to compensate for all our needs if we don\u2019t focus on programs that promote healthy aging for all of our patients,\u201d he said.<\/p>\n<p>The task of improving how we age has received global attention through the <a id=\"\" href=\"https:\/\/www.decadeofhealthyageing.org\/about\/about-us\/what-is-the-decade\" target=\"_blank\" rel=\"noopener\">United Nations Decade of Healthy Ageing<\/a> initiative, which aims to strengthen long-term care, integrated care and \u201cage-friendly environments,\u201d while combating discrimination against people as they grow older.<\/p>\n<h2><strong>What are practical steps people can take to prevent frailty?<\/strong><\/h2>\n<p>\u201cThe core of treatment for frailty is exercise and nutrition,\u201d Johnson said. He emphasized that exercise must include balance training and resistance, such as lifting weights and performing chair lifts.<\/p>\n<p>\u201cMany patients focus just on walking and aerobic training, which is not enough,\u201d Johnson said.<\/p>\n<p>It\u2019s vital to work on aerobic training, balance and resistance or strength training.<\/p>\n<p>Physical and occupational therapists can help patients develop a plan to improve their strength, stamina and their ability to function safely, Johnson said.<\/p>\n<p>For people on certain <a href=\"https:\/\/www.uchealth.org\/billing-and-pricing\/insured-patients\/#medicare-advantage\">Medicare Advantage programs<\/a>, the free exercise program <a id=\"\" href=\"https:\/\/tools.silversneakers.com\/\" target=\"_blank\" rel=\"noopener\">Silver Sneakers<\/a> is \u201can outstanding resource,\u201d Johnson said.<\/p>\n<p>The program offers in-person and virtual exercise sessions. People age 60 and older who live in the city of Denver, for example, have free access to all Parks and Recreation facilities, some of which offer Silver Sneakers classes, Johnson said.<\/p>\n<p>Other exercise options include <a id=\"\" href=\"https:\/\/www.va.gov\/GERIATRICS\/pages\/gerofit_Home.asp\" target=\"_blank\" rel=\"noopener\">GeroFit<\/a> for Veterans and a variety of programs available to people through their health insurance plans.<\/p>\n<p>\u201cMany insurance programs are realizing the benefits of supporting people in their journey to healthy living and aging,\u201d Johnson said. \u201cCheck your insurance for built-in benefits.\u201d<\/p>\n<p>For nutrition, the key to warding off frailty is getting enough protein in the diet, Johnson said. He advises people to work with their providers on recommendations that meet their specific needs, such as managing diabetes or heart disease, but in general, a Mediterranean diet is healthiest. <a id=\"\" href=\"https:\/\/culinarymedicine.org\/moodle\/upload\/HmFHandouts\/Mediterranean%20Diet.pdf\" target=\"_blank\" rel=\"noopener\">Learn the basics about a Mediterranean diet and how to get started.<\/a><\/p>\n<p>Another key resource for older people is the free <a id=\"\" href=\"https:\/\/www.medicare.gov\/coverage\/yearly-wellness-visits\" target=\"_blank\" rel=\"noopener\">Medicare Annual Wellness Visit<\/a>, said Johnson. He called the visit an opportunity for people to think about healthy aging activities and their overall health status.<div class=\"su-pullquote su-pullquote-align-right\"><a href=\"https:\/\/www.uchealth.org\/today\/uchealth-know-your-medicare-and-keep-costs-down\/\">Understand Medicare coverage<\/a> and keep your costs down.<\/div>\n<p>\u201cNext time you have that visit, if frailty is something of concern, bring that up with your primary care physician,\u201d Johnson said. \u201cYour PCP will know the best steps for you as a patient.\u201d<\/p>\n<p>It is also important for people to review the medications they take, Johnson said. Some can make it harder to grow muscle mass or to function physically and mentally. With the help of a doctor and pharmacist, patients may be able to get off some medications, decrease their dosage or find alternatives that cause fewer symptoms.<\/p>\n<h2><strong>Frailty increases the risk of injury-related falls. What efforts at UCHealth and the University of Colorado Anschutz School of Medicine address fall risk?<\/strong><\/h2>\n<p>There are several efforts that address fall risks. Johnson directs the <a href=\"https:\/\/www.uchealth.org\/today\/fall-prevention-clinic-all-about-keeping-people-upright\/\">Fall Prevention Clinic<\/a> at the UCHealth Seniors Clinic, which serves people who have fallen or are at risk for falling. The clinic takes a team approach to fall prevention. Geriatricians, physical therapists and psychologists evaluate and treat patients on the same day of their visit and develop a plan to reduce the risk of future falls, Johnson said.<\/p>\n<p>In addition, Johnson leads the University of Colorado\u2019s participation in the statewide <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/center-on-aging\/outreach-and-engagement\/colorado-statewide-falls-coalition\" target=\"_blank\" rel=\"noopener\">Colorado Falls Prevention Coalition<\/a>, along with <a id=\"\" href=\"https:\/\/www.ucdenver.edu\/change-makers\/people\/jodi-waterhouse\" target=\"_blank\" rel=\"noopener\">Jodi Waterhouse<\/a>, director of strategic partnerships and planning for the <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/center-on-aging\" target=\"_blank\" rel=\"noopener\">Multidisciplinary Center on Aging<\/a>. The coalition includes both health care experts and community members who aim to bolster fall prevention resources throughout Colorado.<\/p>\n<p>Meanwhile, the Administration for Community Living late last year awarded the University of Colorado Anschutz School of Medicine a <a id=\"\" href=\"https:\/\/news.cuanschutz.edu\/emergency-medicine\/goldberg-kwan-levy-step#:~:text=The%20STEP%20program%20in%20Colorado%20is%20a,groups%20across%20Colorado%20*%20UCHealth's%20falls%20clinic\" target=\"_blank\" rel=\"noopener\">four-year, $4.7 million grant<\/a> dubbed <a id=\"\" href=\"https:\/\/medschool.cuanschutz.edu\/center-on-aging\/outreach-and-engagement\/scaling-trusted-evidence-based-prevention-(step)\" target=\"_blank\" rel=\"noopener\">Scaling Trusted Evidence-based Prevention<\/a> (STEP). The initiative focuses on a trio of programs:<\/p>\n<ul>\n<li><a id=\"\" href=\"https:\/\/www.uchealth.org\/services\/community-health\/stepping-on-program\/\" target=\"_blank\" rel=\"noopener\">Stepping On<\/a>, a seven-week program for people who live at home and are at risk of falling<\/li>\n<li><a id=\"\" href=\"https:\/\/northwestcoloradohealth.org\/what_we_do\/prevention-services\/aging-well\/matter-of-balance.html#:~:text=A%20Matter%20of%20Balance:%20Managing,Donations%20are%20appreciated.\" target=\"_blank\" rel=\"noopener\">A Matter of Balance<\/a>, an eight-week program that aims to increase strength and balance and reduce people\u2019s fear of falling<\/li>\n<li><a id=\"\" href=\"https:\/\/coloradolab.org\/2025\/01\/13\/evidence-supports-making-capable-a-colorado-medicaid-benefit\/\" target=\"_blank\" rel=\"noopener\">CAPABLE<\/a>, a program led by a nursing, occupational therapy and handy worker team that conducts weekly home visits to help people improve their mobility and make them more independent.<\/li>\n<\/ul>\n<h2><strong>What is a good way for people to think about frailty in everyday terms?<\/strong><\/h2>\n<p>\u201cI think of frailty as a bridge,\u201d Johnson said. \u201cSomething like the Golden Gate Bridge works well when there is a big storm or earthquake. It\u2019s still safe to travel on it, even in those stressful environments.\u201d Likewise, a person in good health has the strength and reserves to withstand and recover from even serious health challenges.<div class=\"su-callout-box col-xs-12 col-sm-6 right\" style=\"background-color:#dce4e7; color:#2e3b44;\">From alcohol use and supplements, here is <a href=\"https:\/\/www.uchealth.org\/today\/keys-to-longevity-for-older-adults-how-to-age-well\/\">what you need to know to age well<\/a>.<\/div>\n<p>On the other hand, \u201ca person who has frailty is like a bridge that has wear and tear on it and has not been maintained well,\u201d Johnson said. \u201cThat bridge may be safe to travel across on a nice, sunny day. However, if it has enough fraying wires or structural supports with cracks, it would not be safe to travel across that bridge during a storm anymore. You can think of frailty like a bridge in that respect.\u201d<\/p>\n<p>In the everyday world, a problem like a <a href=\"https:\/\/www.uchealth.org\/diseases-conditions\/urinary-tract-infections\/\">urinary tract infection<\/a> for many people requires only a round of medications, taken at home, before they can return to normal, Johnson said. A frail patient with the same kind of infection might be hospitalized with a long illness and recovery.<\/p>\n<p>\u201cThat patient might take some time after the illness to return to their previous function, if at all,\u201d Johnson said.<\/p>\n<p>\u201cIn general, the best way for people to treat the risk of frailty is by working with their primary care provider and by engaging in the healthy habits you\u2019ve been taught your whole life,\u201d he continued. \u201cIf you are frail, push yourself farther and dramatically increase your exercise and <a href=\"https:\/\/www.uchealth.org\/services\/nutrition-services\/\">nutritional support<\/a>.\u201d<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Millions of people in the U.S. who are ages 65 and older are officially considered &#8220;frail,&#8221; and due to demographic trends, the number of vulnerable people is rising fast.
